Content is good, eBook formatting stinks!

First, the content is very good and has helped me learn MDX. The author starts with the very basics of MDX and then builds on it to help you build more complex queries.
My only complaint is with the formatting of the ebook, which is an example of poor eBook conversion to digital media.

The Table of Figures is just blank lines.
THE EXAMPLE CODE IS HUGE IN THE VIEWER.

    The tables
    _____________
    end up
    _____________
    all over.
